Announcements

  • We are in need of at least 8 boys in grades 5-8 to commit to joining our basketball team by this weekend.  If we do not have enough then the season will be canceled unfortunately. Please register as soon as possible!
  • Boys basketball team will practice ona Tuesdays, Thursdays and Fridays from 3:30 to 5:30pm. Practices and home games will be played at Homyak PK-8 School, 3927 York St, Dacono, CO 80514
  • Sports physicals are required for all students who plan to participate on any of our sports teams.
  • Advanced Urgent Care partners with local schools and donates $10.00 to athletic departments when we have 15 or more students have their physicals completed there. Click the link to schedule your appointment! Please don’t forget to tell them the name of our school.

How to Register

Winter Sports Registration

Starts September 8th and Ends on October 11th

High school registration: $125 per student, or $325 family max

Middle school registration: $125 per student, or $325 family max

**Families who qualify for other available benefits should complete the registration process and then contact our school office for additional details.**

Refund Policy: If your child chooses to participate on one of our sports teams and then discontinues their participation for any reason…

1. We will refund 100% of the athletic fee only during the first week of the scheduled season.

2. After the first week, there will be NO REFUND for any reason.

Registration Information

Click the online registration button below to register your child(ren) for athletic participation.

All students in Grades 5 through Grade 9 are eligible to join any of our sports teams. There are NO high school sports for the winter season!

The other buttons will provide key details for each sport level.

Athletic Forms

These forms must be completed prior to the start of each season. The deadline for submitting these forms is October 17.

Athletic Participation

***REQUIRED***

This form confirms that parent(s) are giving full permisison for athletic participation.

CHSAA Sports Physical

An annual physical is required for all athletes. Have your doctor complete all 4 pages. The school ONLY needs page 4 of the completed physical. **We recommend that you consider scheduling your child’s physical for the month of August. This way you will know that your child is covered for the entire school year.

Medical Consent

***REQUIRED***

This form gives permission for any necessary medical treatment for any injury or illness during athletic participation.

Carpool Permission

Parents complete this form only if you plan on having your child(ren) riding with another adult.

Parent Authorized Driver

Parents, please complete if you are wiling to transport other students besides your own.

Student Insurance Verification

***REQUIRED***

This form confirms that each child who participates in athletics is covered by their family’s medical insurance policy.
